Referrals

A client can self-refer over the phone or ask their general practitioner (GP) or other health specialist to send a referral. Referrals must come through the Referral Coordination Centre. End of Life funding applications are faxed directly to the number on the application form.

Referral is only through your GP or hospital service.
All referrals must come through the Community Regional Referral Centre.
Self-referrals are not accepted.

Referrals

Child Development Centre (CDC) accepts referrals from health services, education services and community/welfare agencies. The referral must be written with the knowledge and consent of the child’s parents/carers.

Referrals must be in writing, clearly describing the developmental concerns and supported with any other reports or information.
