Acute Home Support

Acute Home support services is provided only for clients during their acute recovery period from recent surgery or an acute medical illness, or with a palliative illness. It is a short term service providing personal care and home help support for clients in their own homes.

Clients are referred by health professionals, assessed by the clinical nurse coordinator by phone, or by district nurses or hospice nurses visiting the clients at home. Following the assessment process the clinical nurse coordinator develops a package of care and refers to outsourced home support agencies to provide the service.
